A fissure in ano, also known as an anal fissure, is a small tear or crack in the moist, thin tissue (mucosa) that lines the anus. I can cause significant pain, especially during and after bowel movements. Bad diet habit leading to hard stool can further exacerbate the fissure problem.Fissures can be acute (recent onset) or chronic (lasting longer than six weeks). Symptoms:The most common symptoms of an anal fissure include Pain during bowel movements: Often described as sharp, tearing, or burning. This pain can be severe. Pain after bowel movements: This pain can last for several minutes to several hours .Bright red blood on the stool or toilet paper: This is usually a small amount. A visible tear or crack in the skin around the anus.A small lump or skin tag (sentinel pile) near the anal fissure, often seen in chronic cases. Itching around the anus.Spasm of the anal sphincter muscles Causes:Anal fissures are often caused by trauma to the anal canal. Common causes include:Passing large or hard stools: This is the most frequent cause.Constipation and straining during bowel movements. Chronic diarrhea: Frequent bowel movements can irritate the anal lining.Childbirth: Increased pressure during delivery can sometimes cause a fissure. Reduced blood flow to the anal area (more common in older adults).Tight anal sphincter muscles: Increased tension can make the anal canal more susceptible to tearing. Inflammatory bowel disease (IBD) such as Crohn's disease or ulcerative colitis can make the anal lining more fragile. Infections (rare), such as sexually transmitted infections. In some cases, the exact cause of an anal fissure cannot be identified.The goal of treatment is to relieve pain, heal the fissure, and prevent recurrence. Treatment options vary depending on whether the fissure is acute or chronic.Initial and Home Treatments: Dietary changes: Increasing fiber intake (fruits, vegetables, whole grains) and drinking plenty of fluids helps to soften stools and make bowel movements easier. Aim for 25-35 grams of fiber daily.Sitz baths: Soaking in warm water for 10-20 minutes several times a day, especially after bowel movements, can soothe the area, relax the anal sphincter, and promote healing.Avoid straining: Take your time during bowel movements and avoid straining.Gentle cleaning: Clean the anal area gently after bowel movements, using mild soap and water or baby wipes instead of harsh toilet paper. Topical treatments: prescription creams or ointment.Medical Treatments:If home treatments are not effective, a healthcare professional that to be taken after complete chekup only.Laxatives: Bulk-forming laxatives or osmotic laxatives can help soften stools. Surgery is usually considered for chronic anal fissures that haven't healed with other treatments. The most common surgical procedure is Lateral internal sphincterotomy (LIS): This involves making a small cut in the internal anal sphincter muscle to reduce tension and spasm, allowing the fissure to heal.
